There are a few scenarios that could explain why you did not receive credit for completing the Health Assessment.


  1. Ensure you submit your final answers- At the end of your assessment, you will be prompted to select the green checkmark, which will submit your final answers. You will receive an Assessment Complete notification as confirmation of your participation.
  2. Your Program does not award points for the Assessment - The Health Assessment may be a requirement to unlock the incentive offered but does not award credit. Check your challenge details for more information.
  3. Your Health Assessment was completed outside of the challenge's start and end dates - Your submission will not be recognized as complete if your Health Assessment was submitted outside of the challenge timeframe. Please review the challenge details page for these dates.
